Nav-CARE Program - Trail Area

A free, innovative volunteer outreach program designed to provide support to those living with declining health due to life-limiting or chronic illness.

 A trained volunteer navigator helps the person and family to connect with resources, navigate through changes and challenges, learn about the health related condition and anticipate future needs.
