Greater Trail Hospice Society
Offers support and services for those with life-limiting illness or in need of end-of-life care as well as for those who are journeying through grief and loss. The goal of all care is to help the person live as independently as possible, with the best quality of life, for as long as possible.
All services are free of charge.
Hospice staff and well trained volunteers provide confidential resources and support in homes, hospital, long term care or residential facilities.
Programs include:
Volunteer Companioning Programs: Volunteers are able to listen, offer practical assistance, companionship and information, act as an advocate, sit with a loved one at end of life and provide respite for family caregivers.
Navigator (NavCARE) Program: for those who have been diagnosed with life-limiting or chronic illness. A trained volunteer helps the person and family to connect with resources, navigate through changes and challenges, learn about their condition and anticipate future needs.
Bedside Volunteer Program: Provides trained volunteers to sit with individuals in the last hours of their life and to provide loved ones with some respite and support during that time.
Friendly Visitor Program: Offers visits to those who live in supportive residential facilities and who may have few family or friends.
Advance Care Planning: Helps people prepare documents that clearly states wishes about health and end of life care.
Grief and Loss Program: Offers one-to one and/ or group support for both adults and children facing significant loss in their lives.
Library: Many books available for loan on end of life care or grief for adults and children.
Caregivers and clients are invited to contact Hospice at any time for information, support, to self-refer or to sign up for the Monthly Newsletter
A link to a referral form is posted on the Home page of the Trail Hospice website. It can be completed by the client or by family, a physician, home care provider or hospital staff.
