Greater Trail Hospice Society

Offers support and services for those with life-limiting illness or in need of end-of-life care as well as for those who are journeying through grief.

Hospice staff and well trained volunteers provide confidential resources and support that may include:

  • Assessment of client and family needs for support or education
  • Bedside companionship or family respite
  • Integration and coordination of care services
  • One on one grief support
  • Adult Grief Support Group, on the 2nd and 4th Tuesday of each month
  • Children’s Grief Group twice annually
  • Extensive resource library of books, DVDs, etc
  • Community education about end-of-life issues
  • Assistance with Advanced Care Planning

Caregivers and clients are invited to contact Hospice at any time for information, support or to self-refer. 

A referral form is posted on this website and also on the Trail Hospice Website. It can be completed by the client or or by a physician, home care provider, or hospital staff. 

All services provided by the Society are free.
