Community Connector - Nelson Area

A free service that helps to connect older adults with non medical community services and activities that will help reduce isolation and improve a senior's physical, mental and emotional health and well-being

Works with seniors who are experiencing, or at risk of, the following and who wish to become more engaged with the community.

• Frailty
• Social Isolation and/or Loneliness
• Barriers to Accessing Community-Based Resources
• Frequent Trips to Emergency Department or Primary Care
• Experiencing Life-Limiting Illness (Congestive Heart Failure, Renal Failure, COPD, Diabetes, etc.), End-of-Life, or Palliative Care

Helps assess their needs and connect them with the appropriate community resources.

Includes support to identify goals, co-creation of a personalized wellness plan, and connection to services that can enhance wellbeing.

Serves residents of Nelson, Balfour, Bonnington, Taghum, Beasley, Blewett or South Slocan
