Community Connector - Slocan Valley & Slocan Lake Areas

A free program that uses Social Prescribing to offer non-medical support that assists older adults to identify and achieve personal health and wellness goals. 

Helps to find appropriate local programs and services followed by regular check-ins to ensure participants are on track.

Eligibility:  

• 65 years of age or older
• Residingin the area from Crescent Valley to New Denver/Hills

Experiencing, or at risk of:

• frailty; social isolation and/or loneliness;
•  barriers to access community-based resources; 
• frequent trips to primary care or ER

Referrals :

• Self-referral
• Friends, Family, & Community Members
• Health Care Providers
