Community Connector - Lower Columbia (Trail Area)

A free service that helps to connect seniors (65+) with non medical community services and activities that will help reduce isolation and improve a senior's physical, mental and emotional health and well-being.

The Connector :

  • Listens to understand situation of senior
  • Helps to find and access the right supports
  • Keeps connected  
  • Is someone to talk to confidentially. 
  • Is practical, helpful, and will not judge
  • Helps senior decide how to improve their wellbeing. 

A Community Connector can help with various situations, including:

  • Loneliness/isolation
  • Difficulties navigating social systems and community supports
  • Grief/bereavement
  • Caregiver supports
  • Transportation
  • Food Programs
  • Providing information on a wide range of resources including mental health/practitioners, housing, benefits and more.

Referrals can be made by healthcare professionals, community partners, family members, or through self-referral.
