Community Connector - Lower Columbia (Trail Area)
A free service that helps to connect seniors (65+) with non medical community services and activities that will help reduce isolation and improve a senior's physical, mental and emotional health and well-being.
The Connector :
- Listens to understand situation of senior
- Helps to find and access the right supports
- Keeps connected
- Is someone to talk to confidentially.
- Is practical, helpful, and will not judge
- Helps senior decide how to improve their wellbeing.
A Community Connector can help with various situations, including:
- Loneliness/isolation
- Difficulties navigating social systems and community supports
- Grief/bereavement
- Caregiver supports
- Transportation
- Food Programs
- Providing information on a wide range of resources including mental health/practitioners, housing, benefits and more.
Referrals can be made by healthcare professionals, community partners, family members, or through self-referral.
