Community Connector - Nakusp Area (Arrow Lakes)

A free service that helps to connect older adults with non medical community services and activities that will help reduce isolation and improve a senior's physical, mental and emotional health and well-being.

Also offers ongoing support in navigating systems to improve health and well being

Serves: older adults who are experiencing, or at risk of: frailty, social isolation and/or loneliness; barriers to accessing community based resources; frequent trips to primary care or ER and who live in Nakusp, Summit Lake, Burton, Fauquier or Edgewood

Referrals can be made by healthcare professionals, community partners, family members, or through self-referral.
