Youth Intensive Substance Use Services (IHA) - Trail Area
Provides services for youth aged 12 - 24 with complex substance use (addictions) concerns.
Services include:
Youth Intensive Case Management (ICM) team (Nurse, Case manager, Support worker ): works with youth with significant substance use concerns and/or concurrent disorders (substance use and mental illness) or co-existing functional impairment, who face complex challenges accessing existing health or social services.
Mandate is to improve health, social functioning and access to care.
Wrap around service provides assertive, strength based and individualized care that meets the needs of clients in community with the goal of reducing the need for hospitalizations through improved community engagement and support.
Youth Substance Use Connections worker: Provides brief intervention and service navigation for those with complex substance use concerns and supports the transition of youth from acute services into community including bridging into the ICM team, Child and Youth Mental Health Services and/or Freedom Quest.
