Salmo Community Connector
A free service that helps to connect older adults with non medical community services and activities that will help reduce isolation and improve a senior's physical, mental and emotional health and well-being.
Also offers ongoing support in navigating systems to improve health and well being
Serves: older adults in the Salmo area who are experiencing, or at risk of: frailty, social isolation and/or loneliness; barriers to accessing community based resources; frequent trips to primary care or ER
Referrals can be made by healthcare professionals, community partners, family members, or through self-referral.
