Aboriginal Patient Navigator - West Kootenay Boundary Region

Aboriginal Patient Navigator (APN) services: 

Helps understanding of the hospital care system:  Provides support to patients, caregivers and their families while in the health care system.
 
Helps build spiritual connection:  Provides culturally sensitive health care to assist the Aboriginal patient and the health care provider.

Helps promote access to community services: Assists Aboriginal patients' access to community services that will enhance their care and provide patients with different resources.
 
Helps link together community and hospital:  Provides linkage for Aboriginal patients', caregivers, and their families to local health care within the region.
