Self-Management - Chronic Disease Management

Offers free on-line and telephone support self-management programs throughout BC to help adults manage various chronic health conditions including diabetes, chronic pain, cancer, heart, kidney and lung diseases, arthritis, lupus, Parkinson, MS and so on.

New Self-Management Programs:

The Tool Kit for Active Living with Chronic Conditions Program is an evidence-based program that includes all tools in the Chronic Disease Self-Management Program packaged into a one-time mailing. It offers an alternative method for those who cannot or do not wish to attend a CDSMP workshop online.

Tool Kit for Active Living with Chronic Conditions Program with Telephone Calls: Participants in this 6-week program receive the materials from the Tool Kit for Active Living with Chronic Conditions Program and participate in a weekly 30-minute, small group conference call with a Program Leader and other participants.

The Better Choices, Better Health Online Self-Management Program  is a web-based, 6-week program in which groups of about 25 people with a variety of ongoing health conditions participate together . See the website or phone for more information.

The Self-Management Health Coach Program provides individualized telephone support for people living with chronic diseases.  Volunteer health coaches provide weekly 30 minutes telephone coaching for 3 – 6 months for individuals wanting to improve their confidence and motivation to become healthier and to better manage the daily challenges of living with chronic conditions. Individuals are paired with coaches based on gender and possibly shared chronic conditions. This program is available to every British Columbian.
