Self-Management BC: Chronic Disease Self-Management Program (CDSMP)

Offers free on-line and telephone support self-management programs throughout BC to help adults manage various chronic health conditions including diabetes, chronic pain, cancer, heart, kidney and lung diseases, arthritis, lupus, Parkinson, MS and so on.

Self-Management Programs are available for persons with Chronic PainDiabetes,  Other Chronic Conditions and Cancer . Program formats include:

  • Self Study
  • Telephone group
  • Virtual Group
  • In-Person
  • Telephone Peer Health Coach
  • Online program

These programs are also available for Indigenous, Chinese and Punjabi communities. See the website for coordinators' contact information.
