Chronic Disease Management

Offers free community group, on-line and telephone support self-management programs throughout BC to help adults manage various chronic health conditions including diabetes, chronic pain, cancer, heart, kidney and lung diseases, arthritis, lupus, Parkinson, MS and so on.

Community Group Self-Management Programs:  Participants meet as a group (10 to 16 persons) once a week for 2 and ½ hours for six consecutive weeks. People can register for a program by registering on the website, emailing or by phone.

The Self-Management Health Coach Program provides individualised telephone support for people living with chronic diseases.  Volunteer health coaches provide weekly 30 minutes telephone coaching for 3 – 6 months for individuals wanting to improve their confidence and motivation to become healthier and to better manage the daily challenges of living with chronic conditions. Individuals are paired with coaches based on gender and possibly shared chronic conditions. This program is available to every British Columbian.
